What Topiramate is used for:
  • Topiramate is a medicine that is used for the treatment of the following indications:
  • certain types of seizures (partial onset seizures and primary generalized tonic-clonic seizures) in adults and children 2 years of age and above
  • in combination with other medications in the treatment of some types of seizures (partial onset seizures, primary generalized tonic-clonic seizures, and seizures associated with Lennox-Gastaut syndrome) in adults and children 2 years of age and above
  • prevention of migraine headaches in adults and adolescents 12 years of age and above
Do not take Topiramate:
  • If you are allergic to Topiramate, or any of the ingredients in this medicine.
  • Tell your doctor about the allergy and what signs you had.
Warnings and Precautions:
  • Stop the medication right away and talk to your doctor or pharmacist if you develop allergic reactions such as shortness of breath, skin rash, swelling of the face or neck, decreased blood pressure and dizziness. 
  • This medicine may cause serious eye problems like an obstruction of fluid in the eye causing increased eye pressure (secondary angle closure glaucoma) and sudden decrease in vision with or without eye pain and redness.
  • These eye problems may cause permanent vision loss if not properly treated.
  • Contact your healthcare provider immediately if you experience any new eye symptoms, including any new vision problems. 
  • This medicine may decrease sweating and may cause fever (increased body temperature).
  • You should be monitored for signs of decreased sweating and fever, especially in hot temperatures.
  • This is especially important in children. 
  • Some patients may necessitate hospitalization if this happens. Contact your healthcare provider immediately if you experience decreased sweating, a high fever, or a fever that does not go away. 
  • This medicine can cause an increase in the level of acid in your blood (metabolic acidosis) which may lead to kidney stones, soft or brittle bones (osteomalacia, osteoporosis, osteopenia), delayed growth in children, and may cause harm to your baby if taken during pregnancy when left untreated.
  • This medical condition can occur with or without symptoms. Metabolic acidosis symptoms that you may experience include loss of appetite, feeling changes in heartbeat, having trouble thinking clearly and tiredness.
  • Your healthcare provider should ask for a blood test to monitor acidity level in your blood before and during your therapy with topiramate.
  • If you are pregnant, make sure that you talk to your healthcare provider to check if you have metabolic acidosis. 
  • This medicine may cause suicidal behavior or thoughts (like all anticonvulsants) in a small number of patients (1 in 500 patients).
  • Contact your healthcare provider immediately if you experience any of these symptoms, particularly if these symptoms are new, worse, or concern you; suicidal thoughts, thinking about dying, suicidal attempts, agitation or restlessness, difficulty sleeping, new or worse irritability, panic attacks, new or worse depression/anxiety, feeling angry or violent, aggression, acting on dangerous impulses, mania (an extreme increase in activity and talking) and other unusual mood or behavioral changes. However, suicidal behavior or thoughts may be due to things other than medications. Your healthcare provider may check for other factors.
  • Watch yourself closely for any changes, particularly changes, in thoughts, mood, feelings or behaviors.
  • Do not skip your scheduled follow-up appointments with your healthcare provider.
  • Contact your healthcare provider between visits as needed, particularly when you are concerned about your symptoms.
  • Talk to your doctor before starting this medicine if you currently have or have previously had mood problems, depression, or suicidal behavior/thoughts.
  • Talk to your doctor before starting this medicine if you have kidney problems including kidney stones, undergoing dialysis and other kidney problems.
  • Talk to your doctor before starting this medicine if you have previously had metabolic acidosis (high acidity in the blood) or if you have liver problems.
  • Talk to your doctor before starting this medicine if you have decreased bone density or brittle, weak, or soft bones (osteomalacia, osteoporosis, osteopenia).
  • Talk to your doctor before starting this medicine if you have respiratory or lung problems, eye problems (particularly glaucoma which is caused by increased eye pressure), diarrhea, growth problem or a planned surgery.
  • Talk to your doctor before starting this medicine if you are following a ketogenic diet (high in fat and low in carbohydrates).
  • Talk to your doctor before starting this medicine if you are pregnant or plan to become pregnant or if you are breastfeeding. 
  • Do not drink alcohol while taking topiramate. Concomitant use of alcohol and topiramate may increase side effects like drowsiness and dizziness. 
Possible side effects: 
  • Possible side effects include allergic reaction (rare), serious eye problems (like sudden decrease in vision with or without eye pain and redness, new eye symptoms, including any new vision problems), decreased sweating, a high fever, or a fever that does not go away, signs of metabolic acidosis (like loss of appetite, feeling changes in heartbeat, having trouble thinking clearly and tiredness), signs of mood and behavior changes particularly if these symptoms are new, worse, or concern you (like suicidal thoughts, thinking about dying, suicidal attempts, agitation or restlessness, difficulty sleeping, new or worse irritability, panic attacks, new or worse depression/anxiety, feeling angry or violent, aggression, acting on dangerous impulses, mania (an extreme increase in activity and talking) and other unusual mood or behavioral changes),
  • signs of high blood ammonia levels (like worsened mental activities, slowed alertness, tiredness, or vomiting) when taken with a medication known as valproic acid, kidney stones, loss of muscle coordination or dizziness, low body temperature, drowsiness, reduced ability to think, tiredness, concentration and attention problems, confusion, memory or speech problems and decreased alertness.
  • Most common side effects include tingling of the arms and legs, loss of appetite, nausea, taste changes, diarrhea, nervousness, weight loss, speech problems, memory problems, upper respiratory tract infection, tiredness, dizziness, decreased sensitivity or feeling, particularly in the skin, drowsiness, slow reactions, abdominal pain, abnormal vision and fever. 
Other medicines and Topiramate:
  • Tell your doctor or pharmacist if you are taking, have recently taken or might take any other medications including prescription medicines, over the counter medicines, herbals, vitamins and supplements. 
  • If you are taking Valproic acid (anticonvulsant).
  • If you are taking medicines that may affect or decrease your thinking, muscle coordination, or concentration.
  • If you are taking birth control pills. Topiramate may decrease the effectiveness of your birth control pills. Talk to your healthcare provider if you experience changes in your menstrual bleeding when taking topiramate and birth control pills concomitantly. 
  • Talk to your healthcare provider if you are not sure if the medicine you are taking is listed above.
  • Keep a list of the medicines that you take and take it with you during your visits to your healthcare provider.
  • Make sure that you do not start new medicines without talking to your healthcare provider first. 
How to take Topiramate:
  • Take your medication exactly as prescribed by your doctor.
  • Your doctor will determine the dose that you need to take and may change the dose when needed. 
  • Take your medicine before, during or after food.
  • This medicine is intended for oral use only.
  • Swallow the tablet whole. Do not chew the tablet. 
  • Tablets may leave a bitter taste.  
  • Swallow Topiramate sprinkle capsules whole or open the capsule and sprinkle its content on a teaspoon of soft food.
  • Make sure that you swallow the medicine completely by drinking fluids immediately after you eat the food and medicine mixture.
  • Do not chew the mixture of food and medicine. You should not store the mixture for later use. 
  • Make sure that you drink a lot of fluids during the day in order to prevent kidney stones when being treated with topiramate. 
  • Your doctor may ask for blood tests during your treatment with topiramate. 
How to store Topiramate:
  • Keep out of the reach of children and pets
  • Store Topiramate tablets at room temperature (15C to 30C). 
  • Store Topiramate sprinkle capsules at or below 25C.
  • Do not use beyond the expiry date mentioned on the box 
  • Keep medication in its original container (tightly closed) to protect it from moisture and light
  • Do not dispose medication in the wastewater or household waste
