The possibility of excessive hypotension is rare. Caution should be exercised if fluid and sodium depletion occurs. Initiate treatment with caution in patients with heart failure, recent infarction, or those undergoing surgery or dialysis. Angina or acute cardiac arrest may occur after initiating or increasing amlodipine doses, especially with severe obstructive coronary artery disease. It should be used with caution in patients whose kidney function depends on the renin-angiotensin group, such as patients with renal artery stenosis, chronic kidney disease, congestive heart failure, and fluid loss.
